HC, DT, and DPO designed the study. DT and KC analysed the data. TA enrolled patients. HC, DT, DPO, TE, CM, NF, TA, and SV wrote the paper. DT designed, implemented, analysed, and obtained local ethical review board permission for the focus group discussions discussed in this submission. TA performed follow-up of outcomes of patients recruited into the ART programme, and was also one of the clinicians mainly responsible for the consultations in the HIV outpatient clinics and hospital wards.
